Service Description
Program comprised of case managers that support individuals in the community with serious mental illness and co-occurring disorders to maintain and enhance their level of independence and integration in the community. Supports money management education, medication monitoring as needed, assistance with accessing somatic and mental health appointments in the community. Hours: Monday – Friday: 8am – 4:30pm *Closed on some holidays*
